City and State Agencies Utility map request letter with official agency letterhead and project number included. Project boundary map. Prime-Contractors Project letter with official sponsoring agency letterhead stating your firm is the retained firm for the respective project. The letter can be addressed to multiple utilities. Project boundary map. Sub-Contractors Same as Prime-Contractor process; in addition attach a letter from the prime-contractor with official letterhead indicating your firm is a sub-contractor and will be handling the respective project on behalf of the prime contractor.
